Volunteer application forms and reference forms can be downloaded by clicking on the links below. They can be submitted via one of the following options:

1. email to: info@hockeyministries.org (make sure that the subject line reads either:camp application form or reference form.)

2. mail to: Hockey Ministries International

*Note that a first time volunteer must complete an application form and have 2 non-family members complete reference forms and must also submit a criminal record check.

All returning staff must complete the " returning volunteers application form"
